As part of our management team promise to provide excellence in leadership to Holmes Consulting, we are engaging in a 360 process to better understand how our people see us performing.

On the following pages there are statements describing these critical leadership behaviours. Please read each of these statements carefully and make an assessment of the person's performance by providing honest, constructive responses using the 6 point scale described later in this form.
There are no right or wrong answers and remember you have been invited to be a feedback respondent because this person values what you have to say about both their strengths and weaknesses. With this in mind, please avoid using only one part of the scale e.g. all 5's. Instead try to use the scale to differentiate the things the person does well and where they can improve.
You will also be asked for qualitative comments to help the person deepen their understanding of their strengths and weaknesses - please be as specific and descriptive as possible in this section, people always find this feedback insightful and helpful.
