As part of Electrix's commitment to providing high quality leadership, we are looking to understand how we are currently performing in the leadership space.  On the following pages there are statements describing critical leadership behaviours.  Please read each of the statements carefully and make an assessment of the person's performance by providing honest, constructive responses using the 6 point scale described later in this form. Please try not to use only one part of the scale - e.g. all 5s. Instead try to use the scale to differentiate what the person does well and where they can improve. You will also be asked for written feedback - please be as specific and descriptive as possible in this section, people always find this feedback most revealing and helpful.
